Clinical value of screening serum squamous cell carcinoma antigen (SCCA) for HCC

Abstract

Background:Hepatocellularcarcinoma(HCC)isthefifthmostcommonmalignancyinthe world.InEgypt,HCCwasreportedtoaccountforabout4.7%ofchronicliverdisease(CLD) patients.Squamouscellcarcinomaantigen(SCCA)hasbeenreportedtobestronglyexpressed inHCCtissuehamperingitsextensiveuseinclinicalpractice.
Aim:Studythevalueofserumsquamouscellcarcinomaantigen(SCCA)forscreeningof HCC.
Materials&Method:Thestudycomprisedoftwogroups.GroupAincluded60patientswith HCCdiagnosticallyconfirmedbyspiralCT,elevatedalfa-fetoprotein(AFP),and/orliverbiopsy .GroupBCLDpatientsdiagnosedonclinical,laboratory,andultrasonographicalinvestigations; groupBsuclassifiedintothreecategoriesaccordingtoCHILD-PUGHscoreincluded30 patients,matchedforsexandage.Allgroupsweresubjectedtothoroughhistorytaking,full clinicalexamination,andlaboratoryinvestigationsincludingliverfunctions,viralmarkers,and AFPandSCCAestimationusingELISAtechnique.
Results:ThisstudyrevealedahighlysignificantdifferencebetweenpatientswithHCCand CLDregardingserumSCCAlevelsbeinghigheringroupA(10.35±20.677)thangroup B(1.64±2.462)(p=0.02).SCCAlevelwaselevatedinpatientswithHCCwithnormalAFPlevels representingitsusefulroleinearlydetectionandfollow-upofpatientstreatedforHCC.The cut-offvaluewasestablishedat2ng/mlwithsensitivityof80%andspecificityof70%).
Conclusion:SCCAcouldrepresentausefultoolasamarkerfordetectionofHCC.
